In order to address health inequities, the Connecticut Association of Directors of Health developed the Health Equity Alliance (HEA) and a Health Equity Index (HEI). The Index is a web-based data interface that provides scores at varying geographic units of analysis and profiles the relationship between social, economic, and environmental conditions associated with health outcomes.The Index utilizes a series of indicators within nine social determinants, correlated with demographic and health data. Local public health departments, in partnership with community leaders, organizations, and residents, can use this evidence-based information for strategic planning, community assessment, grant writing, and community engagement activities. The intended purpose of HEA is to engage and mobilize community members in identifying and solving health problems, and build capacity for changes in policies and practices to establish healthier communities.
