The HealtheConnections Population Health Improvement team and partners in Cayuga, Cortland, Madison, Oneida, Onondaga, and Oswego Counties worked together to develop the HealtheCNY.org website (“HealtheCNY”). Our goal was to create a central location for health-related data, activities, resources, and best practices to address health issues.
